I'm using the Italian translation patch but in the intro I do not see the subtitles.
the rest works, only in the intro no.

There was some difference in regards to the subtitles with the lowress/highres version of that video. Try switching?
Other than that I don't think I know that patch. Where did you find it?

As per instruction I extracted the .res files in the SS1EE res folder
I do not understand why it does not work

I believe to remember that the subtitles in the intro would only show up if you didn't have sound configured. They weren't meant to be subtitles in the traditional sense, but as an alternative to a missing sound card.

Though, I don't know to which extent this has changed with the EE. Nor do I know whether or how it would be possible to disable sound in the EE.

in this link we see the video where the intro has subtitles but nothing special is specified on how to enable them

I was able to contact the author of the patch and confirmed that the Italian subtitles of the patch do not work.
